Summer + School Year Application

This form is for parents and guardians who wish to apply for the MDO Summer Program AND the following school year. You will receive an informational email as confirmation detailing pricing, curriculum, and what to expect. This form adds you to the waitlist.

You will hear back from the Director when a spot becomes available and may schedule a tour at that time.
